Hepatitis B

If you’ve ever wondered why your baby gets a shot in the hospital before you’ve even picked a name, hepatitis B is usually the answer. It’s one of the first vaccines on the schedule, and there’s a good reason for that. Hepatitis B is a liver infection that spreads through blood and certain body fluids, and in babies and young children, it’s far more likely to become a lifelong infection than it is in adults. Left alone for decades, that can lead to liver damage, liver failure, or liver cancer. This is actually one of the few vaccines that can prevent a type of cancer down the road.

What the vaccine protects against

The hepatitis B vaccine teaches your child’s immune system to recognize the virus before it ever gets a foothold. It doesn’t treat an infection that’s already there, it stops one from taking hold in the first place. That’s the same basic idea behind how vaccines work in general, show the immune system a harmless piece of the virus so it knows how to fight the real thing later. A lot of parents ask about the hepatitis A vaccine around the same time, since the names sound so similar. When kids get it

For most babies, the first dose happens within 24 hours of birth, right there in the hospital. The second dose comes at 1 to 2 months old, and the third rounds out the series somewhere between 6 and 18 months. Three shots, spaced out over the first year and a half or so, and your child is protected.

Here’s something worth knowing if you’re pregnant or recently had a baby. In late 2025, the CDC’s vaccine advisory committee updated its guidance around that birth dose. Babies born to mothers who test positive for hepatitis B, or whose status is unknown, still need that shot within 12 hours, no exceptions there. But for babies whose mothers tested negative during pregnancy, families now have more flexibility to decide together with their pediatrician whether to give the birth dose right away or wait, with the series able to start no earlier than 2 months old if it’s delayed. It’s a genuinely personal decision, and your pediatrician is the right person to talk it through with you based on your own situation.

What side effects look like

Most kids handle this vaccine just fine. The most common reaction is soreness, redness, or a little swelling right where the shot went in, and that usually fades within a day or two. Some babies run a mild fever, seem fussier than usual, or are just sleepier for a bit. Serious reactions are rare. If anything seems off or lasts longer than a couple of days, it’s always fine to call your pediatrician and ask.
